Shipping Recommendations:

✔️ Select Tracked Packet to ensure your device reaches the lab safely and your results are processed without delay. The price ranges from $10 to $15, depending on Canada Post's fuel surcharge.

❌Do not use stamps

 Stamped mail can result in lost or undeliverable packages, with no way to confirm delivery.

Important Reminder:

✔ Be sure to submit your test information via the ONLINE PORTAL. This is the only way your test will be registered and your results processed.

❌ If you skip the form and did not save your device number, the lab has no means of locating your device.

If you’ve lost your return envelope:

✔ Print this form and bring it with you to Canada Post

✔ Place your dosimeter in a sturdy envelope

✔ Complete the customs declaration outlined in Step 1 (this includes the mailing address), and be sure to follow Step 2 for shipping
