🧾 Completing the Lab Online Portal Form
Completing this form is essential. It is the only way the laboratory can link your dosimeter to you (including your name, email, and address).
If this step is skipped record of your device number, the lab cannot analyze your detector. Without the completed form, the sample becomes unidentifiable, such as trying to find a needle in a haystack especially when you did not keep record of your device number. Please ensure to read the instructions below:
STARTING THE TEST:
👉Up to 4 detectors can be submitted on the same form, but they must all be for the same tested address.
👉 If you have detectors for different addresses, you must complete a separate form for each address.
Entering Start Date:
• Under “Test Started At,” enter the exact date and time when you physically remove the detector(s) from the sealed bag and place in position for testing.
• The date and time must reflect the actual start moment. Future dates or times are not accepted.
Once you enter the start date and time, a pop-up window will appear titled “Set reminder to end test (optional)” :
• This is where you can schedule when you want to receive an email notification reminding you to stop your test.
• We recommend also setting a personal calendar reminder.
After having clicked OK, at the bottom of the next page, you will also be asked to specify the level (e.g., basement or ground floor) and the name of the room where the detector is located. Follow the prompts until you reach the confirmation screen.
Depending on your email security settings, you may not receive a confirmation message indicating that the lab has received your information.
In the case that you have not received the email confirmation, do not worry you can verify this yourself:
• Click VIEW TEST RESULTS (also located at the top of the AccuStar Canada page). Enter your 7-digit device number and the tested postal code.
If a form appears showing your name and test details, this confirms that the lab has successfully received your information.
ENDING THE TEST:
• When your test period is complete, you will receive an automated email from the laboratory. This email includes a link to enter your “Test Ended At” date and time. You cannot complete this step in advance.
• If you are completing the form for the first time after your test period has finished, you may enter both the start and end date/time in one submission.

📨 If You Do Not Receive the Reminder Email to end your test:
• Go to: https://accustarlabs.com
• In the bottom right corner, enter your 7-digit device number & tested postal code.
• This opens the Long-Term Test Info page.
• Scroll down to the middle of the page.
• Enter your test end date.
• Complete the math question and click Submit.

It is your responsibility to keep a record of your device number, as we do not retain this information. 🙂
🔐 Important:
If you skip the form and did not save your device number, the lab has no means of locating your device.
